Collaborative Session for ICPF Learning Community
Please join us for our Spring Practice Facilitation Collaborative Learning Session on Tuesday, May 2, 2023, from 12:00 to 12:50 p.m. Pacific Time. This session will bring together members of the practice facilitation community to explore the making of a career in practice facilitation. Speakers will describe different work environments for facilitators, different pathways to facilitation, and share skills, knowledge, and resources.

In her role, Laken uses her clinical and quality improvement knowledge and experience to assist in the creation, enhancement, and implementation of programs designed to improve health outcomes for cardiovascular disease and hypertension control.
E. Dawn Creach, MS | Principal Consultant, CEO/President, Creach Consulting Group
As a sociologist, Dawn brings a systems-level perspective and diverse set of skills to help solve complex problems with the needs of patients, families, providers, and diverse communities at the forefront. Dawn founded Creach Consulting Group in 2017 to focus on her passion of supporting providers and improving patient health through innovative payment models.
